IRWSA Health CommitteeThe 2009 IRWS Health Survey can be found on the OFA web site at offa.org. On the home page you will note drop boxes. Look for the one titled Health Surveys and scroll down to IRWS. It will be available on the site for as long as we want so you have plenty of time to complete your surveys. Take your time and do not submit until you are sure you have answered all the questions pertaining to your IRWS. You can reset a page you are working on if you want to change answers. You can go back to previous pages or quit the survey if you need to attend to something else and come back at a later date. You will notice that there are follow up pages where appropriate. For example, if you answer yes to the health issue cancer or tumors, a follow up page will ask for the specific cancer type.
